Apply for the Momma D Award

Step 1 · Confirm Eligibility

Make sure you're eligible before you start.

This award is open only to current Ethos student-athletes in the 12th grade — high school seniors in the 2026–27 school year — who have completed the full Legacy Plan track. If any of the items below are missing, the application cannot be reviewed.

You must have all of these:

  • Current Ethos student-athlete in the 12th grade (high school senior, 2026–27 school year)
  • Active Ethos Village account in good standing
  • Completed the Ethos Legacy Plan course
  • Earned the Black Apple with Platinum Stem
  • Legacy Plan video uploaded to your Village profile
  • One letter of recommendation ready to upload

Eligibility is verified directly through the Ethos Village platform. If your Village account, course completion, recognition, or Legacy Video cannot be verified as of September 30, your application will not be reviewed. Confirm everything is in order before you submit. Sign in to Ethos Village →

Step 2 · Gather Your Materials

Have these ready before you begin.

You'll be able to save and return to your application, but it's easier to complete in one sitting if you have everything in front of you.

Bring with you:

  • Your Ethos Village username
  • Your school name, grade, and current GPA
  • Parent or guardian contact info (if under 18)
  • Four short-response answers (~150 words each)
  • Your essay draft (750–1,000 words)
  • Letter of recommendation as a PDF or DOCX

Confirmation Email

You'll receive an automated email confirming your application was received. Check spam if you don't see it within an hour.

Eligibility Verification

Ethos staff verify your Ethos Village account, course completion, and Legacy Video against the platform records.

Panel Review

A 5-judge panel scores eligible applications between September 30 and October 5 using the published rubric.

Finalist Interviews

The top 3 finalists are invited to a 30-minute virtual interview with the panel between October 1 and October 5.

Winner Announced

The recipient is announced live on October 19 at the 2nd Annual Ethos Golf Tournament, Brookhaven Country Club.
